Often, home health care is the last step in recovery after a hospital or skilled nursing facility stay. Home care can mean the difference between a patient maintaining independence at home or being admitted or readmitted to a hospital. StoneRISE will be at the center to discuss technical services like wound care, injections and infusion therapy at home as needed and collaborate with physicians to keep patients safe, healthy and at home.
